To assist us in providing a high standard of care, we kindly ask that you upload your health summary below.

Alternatively, please provide your current GP's details so that, in case of an emergency or if additional information is required, we have the necessary context and support to manage your condition responsibly. This is the regular GP you may have, not your Greencare practitioner.

What is My Health Record?
My Health Record is a secure online summary of an individual’s key health information, managed by the Australian Government. It allows healthcare providers, such as doctors, pharmacists, and hospitals, to access and update important medical details, including prescriptions, allergies, immunisations, and past treatments.

Allowing healthcare providers access to My Health Record ensures they have up-to-date and accurate information when making medical decisions.

By granting access to your My Health Record, the practitioner can quickly confirm your diagnosis during the consultation without needing to go through the time-consuming process of manually retrieving your Health Care Summary, which may take up to a week, depending on your GP clinic.

1. Create a My Health Record (if you don’t have one already)

Online Setup: Visit the official My Health Record website (myhealthrecord.gov.au) and follow the instructions to create an account.

Through Medicare: You can also register during a visit to your local Medicare service center.

2. Log into Your Account

Once your account is set up, log in via the My Health Record website or mobile app using your login details.

3. Grant Access to Healthcare Providers

Access Settings: In your account settings, you can control who can view and update your record. You can give your GP, specialists, pharmacists, or other healthcare providers access to your information.

Sharing Preferences: You can choose to allow access to your full record or just specific sections, like your medications or allergies.

4. Opt-In to Participation

If you’re comfortable with sharing your health details, ensure that the option to “allow” access is activated.

You can also manage notifications to stay informed when your record is accessed or updated.

If you have any questions about allowing access to My Health Record, please call our friendly team on 1300 508 888.
